DVA Program

If you are:

  • an Australian Defence Force Veteran or;
  • a war widow,
  • and you have a Gold Repatriation Health Card

Then you may be entitled to receive home care services through the Commonwealth Department of Veterans’ Affairs which funds the Veterans Home Care Program (VHC).

The services provided are very similar to those provided under the CHSP Program and include

  • Domestic Assistance
  • Personal Care
  • Maintenance
  • Transport for Shopping or to Appointments
  • Community Nursing

To access VHC services a comprehensive assessment is conducted by a VHC assessor.  A majority of assessments are done over the telephone.  Individual circumstances are discussed to identify the range of services that may be required. To arrange an assessment for services, phone a VHC assessment agency on 1300 550 450.

Following the assessment, agreed services will be written in a care plan and referred to the agreed service provider, in this case, Hobart District Nursing Service.

Once the referral is received, we will phone to discuss a suitable day and time to provide the service(s) that have been approved in the care plan.
