Staying Active Staying Independent Wellness Program (SASI)

The District Nurses Wellness program SASI aims to keep people active, healthy and independent and able to perform as many different activities of daily life as possible.

The SASI program delivers a home based exercise program, provides falls prevention strategies and advice on managing areas of your health that are affecting your feeling of wellness and ability to remain independent at home.

It involves review by a registered nurse, an exercise physiologist and a community support worker who will work with you to identify your goals, develop an individualised program and support you to achieve those goals.

When we have received your referral from My Aged Care, one of our staff will make a time to talk to you.

Our SASI team will develop a care plan with you that will detail the health and wellness goals that you would like to achieve and the program to help you achieve them.
