Then you may be entitled to receive Veterans Home Care (VHC) services or Department of Veterans Affairs (DVA) Nursing through the Commonwealth Department of Veterans’ Affairs.
Veterans Home Care provides services that support eligible Veterans, War Widows and Widowers to remain independent in their own home and include:
A comprehensive assessment is conducted by a VHC assessor. A majority of assessments are done over the telephone. Individual circumstances are discussed to identify the range of services that may be required.
To arrange an assessment for services, phone the VHC assessment agency on 1300 550 450.
Following the assessment, agreed services will be written in a care plan and referred to The District Nurses.
Once the referral is received, we will phone to discuss a suitable day and time to provide the service(s) that have been approved in the care plan.
DVA Nursing supports eligible Veterans, War Widows, and Widowers to continue living in their own home by helping manage clinical or personal care needs including:
You, your GP, treating specialist, or representative can refer directly to The District Nurses.
Our Community Nurse will contact you to assess you in your home, and complete a care plan for you at your first scheduled visit.
